This is the original 1991 issue of the CD-single (which was later reissued) released in a slimline jewel case.
Both tracks were remixed from the album Adventures Beyond The Ultraworld.
The "phase two - the flower blossoms through what seems to be a concrete surface" sample in the intro of track 2 is taken from the acapella of The Warning by Logic.
